About

Jordan Curtis, originally from the eastern plains of Colorado, has been living and serving in the San Luis Valley community since 2010. She attended Colorado State University and received her Bachelor's Degree in Human Development and Family Studies and Masters Degree in Occupational Therapy in 2008. She has been working with children for more than 20 years in various capacities.

Jordan lives with her husband and 4 children in Center, CO. Her favorite pastimes include supporting her kids’ various interests and activities as well as hiking, backpacking, biking, and going on new adventures.

Billing/Insurance

Breaking Trails Therapy accepts Medicaid CHP+ and private pay clients.

Benefits of Private Pay Services:

  • Prior authorization is not needed to receive services

  • Not limited to a certain number of visits

  • Services can be in various locations and with various stakeholders of the clients

Forms of payment accepted include:

  • Credit Cards

  • Cash

  • HSA

Payment is due at the time of services.

At this time only Medicaid insurance is accepted.

Basic Evaluation - $150

Standardized Evaluation - $250

30 Minute Therapy Session - $50

60 Minute Therapy Session - $100

60 Minute Teletherapy Session - $100

60 Minute Home Visit Session - $120

60 Minute Consultation - $60

Invoices/SuperBills

  • Invoices are generated after services are completed that day

  • SuperBills are generated at the beginning of each month for previous months treatments.

Cancellation Policy

All cancellations must be made 24 hours in advance.

No Show Policy

You will be charged for the full treatment session should you happen to miss your appointment without cancellation or notification.
